Forms

The following forms are intended for members of the Msgr Dewson Council of the Knights of Columbus.

• Charitable Request Form – (PDF, 283 KB) To request support from the Knights of Columbus, a member should fill out this form and bring it to the next KofC meeting for submission to the Council.
• Member Interest Form – (PDF, 26 KB) Members can fill this out to inform the Grand Knight and the Council what activities they would be interested in participating in.

The members-only group is hosted through Google groups. Membership in the group is reserved to members of the council and must approved by group leadership.
